Scholarship Recipient Agreement

The Yampa Valley Community Foundation disburses scholarship grants to the educational institution the recipient attends; scholarships are not disbursed to individuals. Students must complete and return the Scholarship Recipient Agreement to YVCF before funds can be distributed. Please fill out for all scholarships you are receiving from YVCF.

I accept this scholarship and understand it is to be used to further my education, to be used toward: tuition, fees (excluding late fees), books and other qualified educational expenses. If, for any reason, I do not continue my education during the period of this scholarship or no longer meet the scholarship qualifications, I will notify the Yampa Valley Community Foundation and arrange for the unused balance of the scholarship to be returned to YVCF.
