
Northwest Colorado Health supports community wellness through a range of health programs and services helping residents of all ages, income levels and cultures live as healthy as possible. Northwest Colorado Health (formerly Northwest Colorado Visiting Nurse Association) was established in 1964 as a Home Health and Public Health agency in Routt and Moffat counties. Their services have grown to include primary care at Community Health Centers in Steamboat Springs and Craig, a wide range of Prevention Services in Routt, Moffat, Rio Blanco and Jackson counties and Home Health and Hospice services in Routt and Moffat counties. In addition to Community Health Centers, Northwest Colorado Health operates Dental Clinics in Craig and Oak Creek, The Haven Assisted Living and Community Center in Hayden, Casey’s Pond Senior Living Community in Steamboat Springs, and the Rollingstone Respite House in Steamboat Springs.
The Yampa Valley Community Foundation holds several funds to support Northwest Colorado Health in their mission to “Improve quality of life for all Northwest Colorado residents by providing comprehensive health resources and creating an environment that supports community wellness.”
Endowment Fund
This endowment fund was established to support our community today, tomorrow, and beyond, ensuring the future of health care in our valley with access to care from beginning to end of life.
Hospice Services of NW Colorado
This fund provides support for Northwest Colorado Health’s Hospice program. The Hospice program believes that everyone with a terminal illness has the right to live with dignity and comfort.
